Family Access to Medical Insurance Security Plan (FAMIS) applications can be obtained at the local Department of Social Services or application can be made by calling 1-866-873-2647.  This program is administered by the Department of Medical Assistance Services for uninsured children up to age 19.  Some co-pays and deductibles applicable and full services as provided by the current Virginia Medicaid program.  Children whose parents are employed with State agencies or parents that have access to local choice state insurance are not eligible for this program.
