(SLH)
State/Local
Hospitalization (SLH)
provides direct payment to hospitals for individuals who are unable to pay for
required medical services.
If you are unable to pay for these services yourself and you live
with someone
who is legally responsible for you, these people also must be unable to pay.
This includes spouses for each other and parents for children and stepchildren
under age 18.
Medical
services covered are limited to:
In-patient
and out-patient doctor visits are not covered.
If you know you are going to be hospitalized or have out-patient surgery, you
can apply for SLH beforehand. Or, you can apply within 30 days of your
discharge or the date services were provided.
If funds are available and you are eligible, you will be covered for 180 days
from the date of application or the date of service.
